Helpful Information for Filing an Application

If you are able, bring this information with you when you file your application:

  • Bank statements (including checking, savings, CDs, IRAs, stocks, bonds, and cash value of life insurance policies) dating back to December or January
  • Copy of the most recent federal income tax returns for each person in the household required to file
  • Social Security statements from all recipients in the household for the preceding year
  • Pension and/or annuity statements for the preceding year
  • Income from interest, dividends, rent, welfare, gifts, capital gains, or any other sources of income
  • Value of personal property (car, truck, boat, etc.)
